Before filing a complaint with the Office of Compliance, we ask that you carefully read all the information on this page to determine with whom you should file your complaint. Although our office will receive a privacy complaint brought against an employee of DHEC, if it is not within our purview of authority, we will refer the complaint to another state agency or local authorities as appropriate. As such, we encourage you to file your complaint with the appropriate authority to expidite action regarding your concerns. This page will guide you through the process.
You may use this form to file a complaint with the South Carolina Department of Health and Environmental Control (DHEC) if you believe that DHEC has violated the privacy or security of health or other confidential information. Please note that complaints for violation of the Health Insurance Portability and Accountability Act (HIPAA) must be filed within 180 days of the time you become aware of the violation. You may file your complaint electronically, or you may call the DHEC Privacy Officer at (888) 843-3718 to file a complaint. If you are submitting a physical copy of DHEC form 2026, please type or print all information clearly, and use additional pages if more space is needed.
