The South Carolina Department of Health and Environmental Control (DHEC) offers the SC State Vaccine Program (State Program) as a supplement to the Federal VFC Program.

The purpose of the State program is to allow non-FQHC/non-RHC providers to serve the "underinsured" child in their medical home. Participation in the State Vaccine Program also allows all VFC enrolled providers to vaccinate certain insured-hardship children.

Providers must be enrolled in the Federal VFC Program as a prerequisite to enrollment in the State Vaccine Program. Providers may opt to participate in the VFC Program only or both the VFC and State Vaccine Programs. A separate enrollment agreement is required in SCI PAS for the VFC and State Vaccine programs to ensure accountability. Enrollment documents for both programs may be submitted to the Immunization Division together for processing.

Please note the following:

The SC State Vaccine Program offers ACIP recommended vaccines with the EXCEPTION of the Human Papillomavirus (HPV) vaccine, which is ONLY available through the Federal VFC Program.

In order to become a VFC provider in SC, providers must serve children within birth through 18 years old population, must be licensed in the state to vaccinate, and cannot be on the current List of Excluded Individuals and Entities as described by on the United States Office of Inspector General.

Follow these steps to enroll in the SC State Vaccine Program:

STEP 1: Review ALL documents and complete online enrollment in SCI PAS. These documents will provide the necessary information for you to understand the parameters of the SC State Vaccine Program. A Doctor of Medicine (MD) or a Doctor of Osteopathy (DO) is required to complete the VFC enrollment documents in SCI PAS. DO NOT FAX OR MAIL ANY ENROLLMENT DOCUMENTS.

NOTE: Deadline to complete the SC State Vaccine Program re-enrollment is Monday June 17, 2013. Providers, who do not complete ALL required enrollment forms by June 17, 2013 in SCI PAS, will jeopardize their ability to order SC State vaccine beyond June 30, 2013.

STEP 2: If you have not already completed the Federal VFC enrollment process, please do so now before completing the SC State Vaccine Program enrollment.

STEP 3: The ESA should log in to SCI PAS using their current individual e-mail address and password. If you have forgotten your password you will need to contact the Division of Immunizations for a password reset. Once you have logged into your account with the temporary password, you will be prompted to change your password with your first log in. The SCI PAS User Guide is available online under the HELP menu to assist you with this electronic process.

STEP 4: The ESA MUST complete the following electronic forms in SCI PAS:

  • South Carolina State Vaccine Program: Provider Enrollment Agreement (DHEC 1230)

Print and retain a copy of all of the completed documents for your office.


Congratulations! You have now completed the enrollment process for the SC State Vaccine Program!

STEP 5: Log in to SCI PAS periodically to check your SC State Vaccine Program enrollment status. The Immunization Division will contact the ESA by email if enrollment documents are considered incomplete and cannot be processed.
