Introduction:

The Department has recognized the need for new Title V application forms to aid in streamlining the permitting process. The final revised application reflects a joint effort with the Air Program, Industry Representatives and the EPA. The application has been approved for use by the EPA and SC DHEC; no further notice is necessary. The Department is allowing a one year grace period from the date the new Title V application forms appear on the SC DHEC web site for a facility to use the former (original) Title V application forms. However, we believe the revised forms will be easier to use and they contain the pertinent information needed to issue a Title V permit; we encourage all facilities to review these revised forms before making the decision to use the former Title V application forms. The Department also recognized the need to create Title V modification forms for Administrative Amendment, Minor and Significant Modifications as well as Operational Flexibility (502(b)(10)). These modification forms standardize the information needed for Department and EPA review.

A question and answer document and a sample completed application are also linked below. The sample application is meant for demonstration purposes, the specific emissions data and regulatory review are artificial.
