All-Health Team
Success Story
Each All-Health Team winner is required to submit a one page “success story” about their project. The success story should contain the issue, intervention and impact of the project. Please see the success story criteria and sample story. Along with the success story, the winner is also required to submit an itemized budget by April 15th outlining how they would utilize the first place award of $3,000 which is announced at the year end celebration in May of each year.
