Monitoring & Evaluation
The South Carolina Online Reporting and Evaluation System (SCORES) was created within the Department of Health and Environmental Control Bureau of Community Health and Chronic Disease Prevention to provide a comprehensive reporting mechanism for usage among the various programs and their respective community partners. The SCORES section designed to capture information relative to healthy eating and active living is formatted to reflect the Options for Action framework.
Options for Action is an integral part of the DNPAO planning and evaluation efforts. By integrating Options for Action into SCORES, South Carolina is able to effectively and efficiently monitor obesity prevention goals and objectives occurring statewide. SCORES allows users to input progress on nutrition, physical activity, and obesity prevention goals and objectives. SCORES also provides information on partners involved in implementation initiatives, the reach of initiatives, and resources leveraged.
SCORES captures information to answer the question of who is doing what and where. It also enables users to input quality improvement data to report the extent of implementation as well as the completion of intermediate steps needed to achieve outcomes. The outcomes monitored by the SCORES system include policy and environmental changes, such as adoption of Complete Streets ordinances, implementation of school gardens, or establishment of local farmers markets.
Information from the SCORES system can be summarized for dissemination. At the local level, information from SCORES can be used to highlight community successes, assist with grant writing, and develop useful reports. This information is also beneficial at the state level to compile County Profile Sheets, develop the State of the State Report, and provide key stakeholder updates.
