Progress Report and Final Completion Report

Please use the following format for Progress Reports as outlined in Section 607.2 of R.61-15.

Each item should be addressed individually:

1. Costs incurred on the project;
2. Construction activity;
3. Program or service activity; and
4. Any deviations from the submitted application, along with supporting documentation.
