Special instructions for reporting in CHESS

Each user must sign a CHESS User Confidentiality Agreement, and the facility's MOA Contact must sign a Memorandum of Agreement for CHESS. To obtain an electronic copy of either the CHESS User Agreement or MOA, please email Ann W. Bell at bellaw@dhec.sc.gov. The signed agreements should be mailed to Ann W. Bell, DHEC CHESS Coordinator, 1751 Calhoun Street, Columbia, SC 29201.
