Online Complaint Form

The Division of Food Protection responds to complaints and food borne illness incidents concerning any of our regulated program areas. All complaints or incidents may be submitted to our office online, by email, phone, fax or standard mail. Only complaints on Retail Food Establishments located in the state of South Carolina can be investigated by SCDHEC Division of Food Protection. Complaints related to facilities located in other states should be directed to that jurisdiction for reporting or action.
