Motoring and Measure (M&M) Data Project
A National Agenda for Children with Special Health Care Needs: Measuring Success for Healthy People 2010. The overall goal of the project is to develop and implement a national strategy for monitoring and reporting progress toward the achievement of community-based, integrated service systems for children with special health care needs and their families as outlined in the Healthy People 2000, Objective 17.20.
The Division of CSHCN is participating in a grant to evaluate community-based, integrated systems of care for special needs children. This program will evaluate and monitor state efforts to establish integrated systems of care, measure and monitor progress, and identify outcomes.
The six performance outcome objectives to be achieved and used in measuring the success of this effort.
- All children with special health care needs will receive regular and ongoing comprehensive care within a medical home.
- All families of children with special health care needs will have adequate private and/or public insurance to pay for the services they need.
- All children will be screened early and continuously for special health care needs.
- Services for children with special health care needs and their families will be organized in ways that families can use them easily.
- Families of children with special health care needs will participate in decision making at all levels and will be satisfied with the services they receive.
- All children with special health care needs will receive the services necessary to make appropriate transitions to all aspects of adult life, including adult health care, work, and independence.

Data Warehouse
A data warehouse will link vital public agency data with private hospital and Medicaid data to achieve the following outcomes in South Carolina:
- The unduplicated number of children with special health care needs, by category documented and reported ;
- The number of children with special health care needs who have a Medical Home documented and reported;
- Coordinated and effective collaboration between public Human Service Agencies in determing goals and establishing planning criteria for future resources;
- Enhanced coordination between Human Service Agencies in the delivery of services to children with special health care needs;
- Production of sound statistical and analytical data to support management decisions;
- A Geographical Information System to answer need and resource distribution by specified geographical and legislative boundaries;
- Identification of the Transition population in South Carolina.
The Data Warehouse will provide exhaustive and comprehensive, statistical and analytical data concerning the children with special health care needs population in South Carolina.
