Home Health Services

Home  Health Services provides professional and supportive health services to persons in their places of residence: (1) to assist them in achieving or maintaining optimal health, (2) to facilitate earlier discharge from hospitals and nursing homes, (3) to prevent or delay institutionalization, and (4) to provide care and comfort to the terminally ill.

Home Health Services provides professional and supportive health services on an intermittent basis, under the direction of the attending physician, to patients who are essentially homebound due to illness or injury.  

In-Home Services

Skilled Nurses

  • Conduct physical examinations
  • Work with doctors to develop an individualized plan of care
  • Teach self-care related to diseases and medications
  • Specialize in various services including: wound care, ostomy care, IV therapy, instruction and management of chronic diseases such as diabetes and congestive heart failure

Physical Therapists

  • Focus on helping regain muscle and joint motion and strength
  • Offer treatment techniques to relieve pain
  • Help improve physical function, including functional mobility

Occupational Therapists

  • Focus on dialy living activities such as self-care and homemaking
  • Teach how to protect joints and conserve energy
  • Assess home to see if adaptive equipment is needed

Speech Therapists

  • Focus on communication skills – speech and language
  • Assess and treat swallowing disorders
  • Provide techniques to improve mental function

Social Workers

  • Assess for mental and emotional adjustment to health problems and provide counseling
  • Assess for financial and environmental concerns impacting health
  • Assist with developing plans of care to meet needs
  • Assist with and teach problem solving skills

Home Health Aides

  • Offer short-term help with bathing and dressing
  • Promote independence in a safe environment
  • Assist in following through with rehabilitation exercises and activities
