Postpartum Newborn Home Visits

Home visits are provided to newly delivered Medicaid mothers.  Self-referrals can be made, but require a physicians order from either a pediatrician or obstetrician.

  • immunizations,
  • limited laboratory testing,
  • WIC certifications,
  • nutrition and social work assessments and interventions, and
  • other activities that promote healthy growth.

Services include:

  • Maternal assessment,
  • Newborn assessment,
  • Limited environmental assessment,
  • Appropriate referrals to needed resources,
  • Return visit if problems are identified for follow-up

Fees: Medicaid is billed for eligible patients.
