Children With Special Health Care Needs (CSHCN) CRS

CSHCN is a group of programs for children with special health care needs serving children to age 18. Any person, agency, or provider may refer a child for services by calling (843) 915-8806. The services provided include care coordination, referrals to community resources, and may include some financial assistance for specialist visits and some medications.

Immunizations

We provide routine childhood immunizations at a public health clinic in each county. We also offer adult vaccinations for flu, pneumonia, tetanus, diptheria, and pertussis (Tdap).

You can get your vaccinations at one of our clinics, through your family physician, or through your child's pediatrician.

Postpartum Newborn Home Visits

A nurse visits with you and your new infant to make sure that both of you are in good health. The nurse offers education, counseling, and referrals. For more information call 843-915-8804 or follow this link for additional Newborn Home Visits information.

 

Nurse-Family Partnership

Are you pregnant with your first baby?  We can help.  A registered nurse can visit you in your home throughout your pregnancy and until your baby turns two.  For more information call 843-915-8899.

WIC

WIC (Women, Infants, and Children)  is a special supplemental food program, federally funded by the USDA to assist states in safeguarding the health and nutritional well being of our low income women, infants and children during critical growth periods. The WIC program is designed to offer a nutrition health assessment, nutrition and breastfeeding education, health referrals, along with supplemental nutritious foods. The program is available to pregnant women, women who have just had a baby, and children from birth to age 5 who are income and nutritionally eligible.
