I. TITLE: Administrative Orders, Consent Orders, and Sanction Letters issued by HEALTH REGULATION (HR).
II. SUBJECT: Sanctions completed during the period May 1, 2010, through May 31, 2010.
III. FACT: For the period May 1, 2010 through May 31, 2010, Health Regulation issued one penalty letter and five (5) Consent Orders with total penalties of $28,000.

IV. ANALYSIS:
- Medxpress Transport, LLC, 525 Dargan Street., Florence, SC 29506. Under a Consent Order executed May 4, 2010, a monetary penalty of $500.00 was paid by the facility. The transportation service was found to have violated R.61-7, Emergency Medical Services, by allowing an uncertified employee to perform patient care for a period of eight (8) days beginning on 6/8/2009 and ending on 7/15/2009.
Previous Sanctions: None.
- AMS of West Columbia, 210 9th Street, West Columbia, SC 29169. Under a Consent Order executed May 25, 2010, a monetary penalty of $500.00 was paid by the transportation service. The facility was found to have violated R. 61-7, Emergency Medical Services, by allowing uncertified personnel to perform patient care.
Previous Sanctions: None.
- Paul Rentz, 140 Dusty Trail Cr., Surfside Beach, SC 29575. Under a Consent Order executed May 18, 2010, Mr. Rentz agreed to surrender his EMT-Paramedic card in lieu of an EMT Intermediate certificate, pending proof of agreed upon training. Mr. Rentz was found to have violated R.61-7, Emergency Medical Services, by failing to perform an adequate patient assessment.

- Amber Diagnostics, 2180 Premier Row, Orlando, Florida 32809. Under a Consent Order executed May 26, 2010, the facility paid $3,000 of a $10,200 civil penalty, with $7,200 stayed upon an eighteen month period of compliance. The facility was found to have violated R.61-64, X-rays, for failure to register as an x-ray vendor.

- Greer Memorial Hospital, 830 South Buncombe Road, Greer, SC 29650. Under a Consent Order executed May 21, 2010, the facility paid $22,500 of a $30,000 civil penalty, with $7,500 stayed upon a period of twenty-four (24) months of satisfactory compliance. The facility was found to have violated R.61-15, Certificate of Need for Health Facilities and Services, for failure to complete projects in accordance with the approved applications and failure to report a substantial change to an approved project.
Previous Sanctions: 2007 - $192,433 – use of unlicensed beds
- J & T Residential Care Facility, 604 Wagon Wheel Road, Hampton, SC 29924. By letter dated April 14, 2010, the Department initiated action to impose a $1,500 monetary penalty against the facility. The facility was found to have violated Section 103 F. of 25 A S.C. Code Ann. Regs. 61-84, Standards for Licensing Community Residential Care Facilities for which the facility was providing supervision to persons in excess of its licensed capacity.
