I. TITLE: Administrative Orders, Consent Orders, and Sanction Letters issued by HEALTH REGULATION (HR).
II. SUBJECT: Sanctions completed during the period November 1, 2010, through November 30, 2010.
III. FACT: For the period November 1, 2010 through November 30, 2010, Health Regulation issued one (1) License Denial and four (4) Consent Orders with total penalties of $24,795.

IV. ANALYSIS:
- Adams Life Link Ambulance, LLC, 1435 N. Longstreet St., Kingstree, SC 29556. Under a Consent Order executed November 8, 2010, a monetary penalty of $300.00 was paid by the company. The company was found to have violated Regulation 61-7, Emergency Medical Services, for failing to provide required electronic run reports to the Department.
Previous Sanctions: None
- Self Regional Healthcare, 1325 Spring St., Greenwood, SC 29646. Under a Consent Order executed November 8, 2010, a monetary penalty of $2,000 was paid by the facility. The facility was found to have violated Regulation 61-15, Certification of Need for Health Facilities and Services, for failure to submit monthly progress reports.

- UniHealth Post-Acute Care – Columbia, 2451 Forest Dr., Columbia, SC 29204. Under a Consent Order executed November 17, 2010, a monetary penalty of $20,995.00 was paid by the facility. The facility was found to have violated Regulation 61-15, Certification of Need for Health Facilities and Services, for not obtaining a Certificate of Need prior to undertaking any activity requiring Certificate of Need review and for violating Regulation 61-17, Standards for Licensing Nursing Homes, for exceeding its licensed bed capacity.

- Knotty Headz II Tattoo Factory (Proposed), 1221 Broad River Rd., Columbia, SC. By letter dated July 22, 2010, the Department, in accordance with S.C. Code Section 44-34-110(A)(1) advised Ernest Comartie III, Esquire, of its decision to deny issuance of a license for the proposed Knotty Headz Tattoo Factory II located at 1221 Broad River R., Columbia, SC due to the facility being located within 1000 feet of a church.

- Presbyterian Home of SC – Florence, 2350 W. Lucas St., Florence, SC 29501. By letter dated October 13, 2010, the Department notified the facility of its decision to impose a $1,500 monetary penalty against the facility for violations of Regulation 61-84, Standards for Licensing Community Residential Care Facilities (Supp. 2009). Violations were beginning and completing construction/renovation projects prior to, or without, the Department's approval.
