I. Title: Administrative Orders, Consent Orders, and Sanction Letters issued by HEALTH REGULATION (HR).

II. Subject: Sanctions completed during the period April 1, 2011, through April 30, 2011.

III. Facts: For the period April 1, 2011 through April 30, 2011, Health Regulation issued one (1) Consent Order with total penalties of $8,700.00.

IV. ANALYSIS: Carolina Home Health Care, 810 Dutch Square Blvd., Suite 206, Columbia, SC 29210. Under a Consent Order executed April 20, 2011, the facility paid a monetary penalty of $8,700. The facility was found to have violated Regulation 61-77, Standards for Licensing Home Health Agencies, for the following:

• Nursing care and therapeutic services were not provided in a safe and effective manner;
• Failure to have resident care plans that addressed physician visits, advanced directives and diet, and were signed by the patient or responsible party; and
• Failure to assess whether home health services were the appropriate type of care for a resident.
