Any business or individual who possesses an X-ray-producing machine must register the machine with DHEC within 30 days of acquiring the machine.

You are also required to notify DHEC any time you sell or expose of a registered X-ray machine.

Moving Machine to New Location? Notify DHEC

Facilities must report to the Department, within thirty (30) days, any changes of status affecting any x-ray machine or facility. If your facility moves an X-Ray machine from one address to another location owned by the same person or people, the report of change status must be made on Form DHEC 4262 “Report of Change”. You will not need to re-register the machine or pay an additional application fee. (However, if applicable, a new shielding plan must be submitted to and accepted by the Department prior to the move.) You may submit the form electronically to: XRayFRA-ShieldingPlans@dhec.sc.gov.

If you are opening an additional or satellite facility, you must complete a facility registration approval request form and submit the application fee. (If applicable, a new shielding plan must be submitted to and accepted by DHEC prior to installation of equipment.) Please refer to the Facility Registration section for more information.
