I. TITLE: Administrative Orders, Consent Orders, and Sanction Letters issued by HEALTH REGULATION (HR).

II. SUBJECT: Sanctions completed during the period June 1, 2010, through June 30, 2010.

III. FACT: For the period June 1, 2010 through June 30, 2010, Health Regulation issued two (2) Suspensions, two (2) Consent Agreements, and two (2) Consent Orders with total penalties of $ 5,750.00.

IV. ANALYSIS:

  1. Carolina Family Care, Inc., 1341 Old Georgetown Road, Suite C, Mount Pleasant, S.C. 29464. Under a Consent Order executed May 18, 2010, a monetary penalty of $2,000.00 was paid by the facility. The facility was found to have violated Regulation 61-15, Certification of Need for Health Facilities and Service, for failure to submit monthly progress reports.

  2. Grady P. Burke, 907 Martin Towne Rd., Ninety Six, SC 29665. Under an Emergency Suspension Order executed June 8, 2010, Mr. Burke’s Emergency Medical Technician Certificate, No. 007714, was suspended immediately for misconduct in violation of Regulation 61-7, Emergency Medical Services, on the basis that he has been arrested for a crime involving child abuse, specifically Criminal Sexual Misconduct With A Minor.

  3. Raymond Pettit, 1250 Jerusalem Rd., Pacolet, SC 29372. Under a Consent Agreement executed June 14, 2010, Mr. Pettit was mandated to surrender his EMT-Paramedic certification and be issued an EMT-Intermediate certification for a period of six (6) months. The individual was found to have violated Regulation 61-7, Emergency Medical Services, by failing in his duty to perform continuous patient monitoring and falsifying documentation. Mr. Pettit must provide proof of successful completion of courses required under the Consent Agreement.

  4. Chris Roscoe, 4496 Turnage Rd., Patrick, SC 29584. Under a Consent Agreement executed June 29, 2010, Mr. Roscoe was mandated to attend remedial training. The individual was found to have violated Regulation 61-7, Emergency Medical Services, by performing an advanced procedure without Medical Control oversight. Mr. Roscoe must provide proof of successful completion of courses required under the Consent Agreement.

  5. Patterson Dental Company – Charlotte, 9129 Monroe Rd., Suite 160, Charlotte, NC 28270. Under a Consent Order executed May 18, 2010, a monetary penalty of $3,750.00 was paid by the facility. The facility was found to have violated Regulation 61-64, X-ray, for repeated failure to meet the service and installation requirements of x-ray equipment as required by regulation.
