DHEC makes health care facility, activity, and professional inspection reports available to you to help you evaluate the quality of care. Generally, these reports are available 30 days after an inspection. Reports for inspections performed during the last 3 years are available. Reports prior to those dates and other public records may be requested by mail, phone, fax, or email by contacting DHEC's Freedom of Information office .

These public record investigation reports are for all health care providers licensed or permitted by DHEC, including:

  • Abortion Clinics
  • Ambulatory Surgery Facilities, Body Piercing Facility (Permitted, not licensed)
  • Birthing Centers
  • Community Residential Care Facilities (CRCF),
  • Day Care Facilities for Adults
  • Freestanding or Mobile Technology
  • Hearing Aid Specialists and Temporary Permit Holders
  • Home Health Agencies
  • Hospice Agencies
  • Hospice Facilities
  • Hospitals
  • Inpatient and Outpatient Treatment Facilities that Treat Individuals for Psychoactive Substance Abuse or Dependence
  • Intermediate Care Facilities for Persons with Intellectual Disability (ICFMR)

  • Perinatal Oversight of Hospitals
  • Renal Dialysis Facilities

Federal Certification Information - Nursing Homes

The Centers for Medicare & Medicaid Services (CMS) Nursing Home Database contains information on every Medicare and Medicaid certified nursing home in the country. Generally, Bureau of Certification reports are available 60 days after the inspection.
